High School Cross Country Sleep Away camp.
In addition to the online registration form, you need to send (or fax) both the parental release form and the medical release form. You may send parental release form with payment, and medical release form at a later date if you wish. We must have both release forms prior to your son or daughter attending camp. The medical release form can be filled out by a physician or school nurse. If your child has had a physical AFTER September 1, 2011, you may use that instead.
